Local Church Survey

Step 1:
Filling out this survey will work best with Adobe Acrobat. If you do not already have access to this program, you can download a free trial version of the software here.
Step 2:
Download Local Church Survey PDF file you will find linked to the bottom of this page.
If you are using Adobe Acrobat (full version): As you work on this survey, keep it saved to your computer. When you are finished answering the questions, go to Step 3.
If you are using Adobe Reader: Complete the survey (you will not be able to save so you must complete the survey all at once) and follow the directions to e-mail a .xml file to aconklin@pnwumc.org
Step 3:
Click the "Submit By Email" button at the top of the form to send in your completed survey to aconklin@pnwumc.org.
(This method is preferred because then your answers can be easily incorporated into the Archives and History electronic records)
